NO CONTRACTS. Month-to-Month.
Most gyms will make you sign a contract. We don’t. At Madrid Jiu Jitsu & Self-Defense, we made a conscious decision to promote the kind of community we believe in — one built on mutual respect, personal growth, and shared values. If you show up ready to train, you belong here. Our job is to help you grow, stay safe, and become your best self — on and off the mats.

Membership Renewal

I, the student (or parent/guardian if under 18), understand that my membership renews every 30 days at the current rate unless I submit a written cancellation request to Madrid Jiu Jitsu & Self-Defense. I acknowledge that a 30-day written notice is required to cancel and that I am responsible for one final billing cycle after the notice is submitted. I remain responsible for all charges incurred prior to cancellation.

Auto-Payment Billing

A valid debit or credit card must be kept on file and will be used to process monthly auto-payments. It is my responsibility to update my billing information as needed. Payments will be automatically withdrawn on my due date.

Non-Transferable Memberships

Memberships cannot be shared or transferred. Each student must have an individual, paid-in-full membership.

Included with Membership

Members receive full access to age-appropriate Brazilian Jiu Jitsu and self-defense classes, as well as belt promotions and tests, provided they meet the academy’s attendance and conduct standards.

I hereby voluntarily consent to participate in the Martial Arts Program at Madrid Jiu Jitsu & Self-Defense, located in Phoenix, Arizona. This includes instruction in Brazilian Jiu Jitsu, grappling, striking, self-defense, fitness conditioning, and related physical training.

I certify that I am in good health and assume full responsibility for any injuries sustained. I understand and accept that martial arts training is inherently risky and may result in minor to severe injuries, including but not limited to sprains, fractures, concussions, or even death.

I understand and agree:

• That participation is voluntary and carries inherent risk.

• That I am using the facilities and equipment at Madrid Jiu Jitsu & Self-Defense at my own risk and “as-is.”

• That Madrid Jiu Jitsu & Self-Defense, its owners, employees, affiliates, and instructors are not liable for any injury, damage, or loss sustained on or off the premises, during training or transportation, regardless of cause.

• That I waive all rights to legal claims against Madrid Jiu Jitsu & Self-Defense, its instructors, owners, affiliates, or third parties acting on its behalf.

• That I authorize Madrid Jiu Jitsu & Self-Defense to use any photographs or video of me for marketing, social media, or advertising purposes.

I agree that any dispute shall be resolved through binding arbitration in Arizona. This waiver is binding upon my heirs, executors, and assigns.

RISK AGREEMENT

I acknowledge that:

• Martial arts training carries physical risk and that injuries, including serious injuries, can occur.

• I am responsible for my safety and that of my training partners.

• I have the right to excuse myself from any drill I believe is unsafe or harmful.

• I must maintain a respectful attitude and follow the instructions of qualified instructors.

PHYSICAL CONTACT NOTICE

I understand and accept that training involves physical contact, including but not limited to grappling, striking, takedowns, and body-to-body pressure. I have the right to withdraw from any training that feels unsafe or inappropriate and should report concerns to an instructor immediately.

CONSENT TO MEDICAL TREATMENT

I authorize instructors and responsible adults to seek medical care in case of injury. I assume all financial responsibility for such care unless caused by criminal negligence.

STUDENT CODE OF CONDUCT

• Show respect to everyone — instructors, training partners, and guests.

• Address all black belt instructors as Professor, others as Coach.

• Minimize unrelated talking during class.

• Arrive on time. If late, wait at the mat edge for instructor approval.

• Place shoes and belongings in designated areas only.

• Do not enter mat space before your class begins.

• Help maintain cleanliness throughout the academy.

• No food or gum in the training areas.

• Only capped water bottles are allowed on or near the mats.

• Electronic devices must be kept off the mats and training areas.

• Foul language and inappropriate behavior are not tolerated.

HYGIENE POLICY

• Wash your gi/uniform after every class.

• Keep nails trimmed and maintain good hygiene.

• No training if you have contagious skin conditions (e.g., ringworm).

• Wear rash guards or t-shirts under gi tops.

• Remove all jewelry and piercings before class.

• Do not attend class if you’re sick or experiencing symptoms.

• Report all injuries or health concerns to an instructor.
